CRN Stories: Real World Impact

CRN stories are real-world examples that show how CRN is changing lives in your community.

“CRN puts my clients at the front of the line in a third of the time!”
-Homeward Bound caseworker

The housing shortage in western Colorado has made it difficult for new moms to find a place to begin their families.

A case worker at the Grand Junction Housing Authority reported that, with CRN’s fast electronic referral system, he was able to find housing for a young pregnant couple in record time. Something that normally took three weeks was accomplished in just a few days.

“CRN puts my clients at the front of the line in one-third of the time!”

Recognized by the ONC!

Quality Health Network’s Community Resource Network (CRN) was recently highlighted in the ONC’s SDoH Toolkit.

As a nationally-recognized trailblazer in the SDoH information exchange space, our CRN platform was called-out as a crucial resource for other health organizations interested in developing similar initiatives in their communities.

The Office of the National Coordinator for Health Information Technology (ONC) is the federal government’s lead agency for healthcare technology. ONC’s SDoH Information Exchange Toolkit also highlighted QHN for our expertise in the technical infrastructure and data standards that makes our whole health information exchange so robust.

QHN is proud to partner with our over 500 healthsocial, and behavioral partners to realize the very best in information exchange standards, and we are excited to continue to find new ways to save our participants time, money, and even lives in our western Colorado community.
