Project 1.27 acts as a bridge between the church community and kids in foster care who need families. Through the training and support of foster and adoptive parents, families are better prepared and encouraged throughout their journey. Project 1.27 also partners with local churches and agencies in order to recruit more families and smooth the way for parent certification.

At Grand Junction Gastroenterology, we’re a group of physicians specializing in the care of digestive and liver related conditions. We are a referral-based specialty clinic. 

Our physicians are board-certified Gastroenterologists.  We do our best to provide advice, testing, and management for outpatient and inpatient adults with symptoms related to the esophagus, stomach, liver, gallbladder, pancreas, small bowel, and colon. We also provide GI related screening and preventative care for adults without symptoms.

Funding Opportunities – 

  • Funding (letter of intent due 3/30; application due 5/1) – Telehealth Remote Monitoring Grant for rural outpatient health care facilities to 1) pay for the acquisition costs of telehealth remote monitoring equipment and devices AND 2) train staff to use, assess, and monitor telehealth remote monitoring equipment and devices. Rural Health Care hospitals, rural health centers, or FQHCs in designated rural counties or designated Federally Qualified Health Centers (FQHCs) are eligible to apply and receive funding up to $100,000.   • Funding (due 4/15/2025) – Black Resilience in Colorado Fund addresses systemic racism and impact on Black communities in the Denver metro area.
  • Funding (due 4/21/2025) – but need to confirm through Jose) – This is a three-year funding opp to reduce disparities in youth cannabis use by increasing protective factors for LGBTQ+ youth in rural Colorado. They encourage creative, innovative, and LGBTQ+ youth-driven proposals. Examples of projects could include:
    • Youth/Community-led projects that increase affirming environments
    • Building public support for district-wide school/community policies that promote safety and belonging (gender-affirming restroom use, GSA policies, pronoun usage, anti-harassment policies, etc.)
    • Institutionalizing LGBTQ+-affirming education across schools
    • Enhancing community coalitions supporting LGBTQ+ priorities
    • Professional development opportunities for youth-serving individuals or organizations
    • Community engagement around LGBTQ+ needs & experiences
    • Implementing needs/readiness assessments
    • Building public support for LGBTQ+ priorities
    • Other low-cost needs that prepare the applicant or community for future
    • LGBTQ+-affirming work aimed at reducing cannabis use

FYI, Mesa is counted as an urban county for this RFA, as are Gilpin, Clear Creek, Park, Teller, and Elbert (plus the traditional metro counties), so you’d need to be serving folks not in those counties. Applicants must be based in Colorado. Priority will be given to applicants geographically located in a rural/frontier Colorado county who have demonstrated expertise working with rural LGBTQ+ youth. Please share to all your rural LGBTQ+ orgs/folks you know. Here is the full RFA. If people have questions, direct them to José Barrón at jose.barron@state.co.us. An informational pre-application webinar to provide an overview of the funding opportunity for potential applicants and help answer questions from applicants will be hosted on Tuesday, March 18, 2025, 10-11 a.m. (MDT).

  • Funding (due 9/30/2025) – Grants to support health programs, average grant size is $10,000 – $20,000. Prioritizes communities in southeast and southwest Colorado.
  • Funding (due 10/1/2025) – The Colorado Department of Public Health and Environment (CDPHE) is pleased to announce the release of an Application for Reimbursement through the Prescription Accessibility Grant Program on December 11, 2024. The Prescription Accessibility Grant Program was created to award hardship grants to pharmacies that face financial barriers to complying with House Bill 24-1115. CDPHE will award approximately $65,000 to pharmacies, with a maximum award of $1,500 per individual grant recipient. Grant funds must be used exclusively to purchase equipment that creates accessible prescription labels. Eligibility for this funding is as follows:
    • Pharmacies registered and based in Colorado.
    • Independent pharmacies
      • An ‘independent pharmacy’ is defined as a prescription drug outlet privately owned by at least one licensed pharmacist with no ownership interest by or affiliation with a chain or publicly owned pharmacy.
    • Ineligible entities
    • Publicly traded pharmacies (Walgreens, CVS, etc) are not eligible for this funding opportunity.
    • Pharmacies that meet eligibility criteria and have purchased labeling equipment that complies with the new accessibility requirements are encouraged to apply for grant reimbursement. The application deadline is October 1, 2025. Review the application requirements and apply for grant reimbursement.
  • Funding (11/1/2029) – Outdoor Recreation Legacy Partnership (ORLP) Program Funding –  The ORLP offers 50/50 matching grants to create new parks or revitalize existing parks in underserved urban neighborhoods. Grants can be used for land acquisition, new park development, or the substantial renovation of existing parks, to (re)connect people with the outdoors.   • Funding (applications accepted on an ongoing basis) – Hearst Foundations Health Grants. The Hearst Foundations provide grants for organizations working to address access to healthcare for high-need populations, healthcare professional shortages, evolving healthcare demands, and medical research.
  • Funding (applications accepted on an ongoing basis) – National Grassroots Organizing Program. Two-year unrestricted, general operating support grants of up to $30,000 per year, with an average grant size of $20,000 per year, to small, constituent-led grassroots organizations throughout the US and its territories.
  • Funding (applications accepted on an ongoing basis) – Economic Development Assistance. Funding to support the implementation of regional economic development, including public works projects and job creation projects. Past projects have included funding for water and wastewater projects in hospitals and healthcare facilities and road infrastructure projects that would support the expansion of healthcare facilities. Rural tribal and other underserved communities are of priority.
  • Funding (applications accepted on an ongoing basis) – Robert Wood Johnson Foundation. Grants for innovative projects that are working to build health equity, ensuring that everyone has access to the care they need and all families have the opportunity to make healthier choices.

  • Training Opportunity for Young Adults: Build a Career in Population Health – HealthTeamWorks Population Health Career Lattice (PHCL) program offers clear, structured pathways for individuals to advance from entry-level to experienced roles in Community Health Work, Care Management, and Clinical Health Coaching. The program is designed to equip participants with essential skills to support their communities while building meaningful, sustainable careers in healthcare. Now available in Colorado, Iowa, and Missouri, the PHCL program is designed to support individuals who are passionate about making a difference in underserved communities. Eligible participants include:
    • Youth aging out of foster care
    • Graduates of alternative schools entering the workforce
    • Young adults (ages 17–24) living at or below the poverty level
    • Individuals committed to serving underserved populations in Iowa, Colorado, and Missouri

  • Webinar Series – Rocky Mountain Health Plans invites you to join us for our Behavioral Health Skills Webinar series. These virtual events are free of cost to attendees and features Colorado-based and national experts. These events are designed for primary and specialty care teams, community mental health and independent behavioral health providers, and community organizations who assist with access to behavioral healthcare. Held on the third Tuesday of each month from 12:00-1:00 PM MST starting in March, these webinars will explore timely and impactful topics in behavioral health.

  • Labor and Sex Trafficking in Colorado: A Training for Healthcare and Behavioral Health Professionals – recording and all resources are here: RMHP Behavioral Health Skills Webinar Trainings.
  • Despite being preventable, tooth decay is the most common chronic disease in children, and is significantly more common than asthma, obesity, and diabetes. Promoting good oral health habits and educating families and children is integral in preventing the potential lifelong consequences of poor oral health. Good oral health habits include brushing twice daily with a fluoride toothpaste, cleaning between teeth regularly (flossing, interdental brushes, water flossers), drinking optimally fluoridated water, and visiting an oral health professional for routine care. Below are some oral health resources available to share with your communities. Please contact the CDPHE Oral Health Unit with any questions or to request additional resources or support.
  • Mobile Public Health Clinic – These mobile clinics provide low- and no-cost services to Coloradans of all ages. These include routine vaccines required for school and child care, as well as other recommended vaccines like flu, COVID-19, and mpox. The exact vaccines offered depend on the specific clinic.
  • Risk and Protective Factors – A table breaking down a multitude of risk and protective factors. Great resource to use when thinking through our work in communities.

  • B:Side Fund Offers long-term financing of up to $350K for for-profit businesses in Colorado, supporting needs such as working capital, debt refinancing, business acquisitions, and equipment purchases.  You can contact B:Side at fund@bside.org. They also offer  B:Side University for Small Business Owners: Helps entrepreneurs develop a well-rounded skill set necessary for the challenges and opportunities they may encounter in managing their businesses. These free, monthly webinars are presented by both B:Side Fund and partners with extensive expertise in their field. Register for the upcoming session “Accounting for small business” HERE.
  • CDPHE houses Get Ahead Colorado / Hacia Adelante Colorado, a tax outreach campaign focused on making sure Colorado families don’t miss out on all the incredible tax credit opportunities available each year. We present and train to public health, early childhood and human services programs. Some examples of the programs we partner with include training 2-1-1 operators statewide, presenting to Family Resource Centers, county human services directors, CCAP, WIC, Connect4Health, Rocky Mountain PBS and many others. This year they are particularly focused on connecting low and moderate income families to the Family Affordability Tax Credit which will deliver up to $3,200 per child to Colorado families, as well as our new tax credit calculator which allows visitors to calculate their potential credits in seconds. We also have a partner toolkit with resources for programs to use to share information.
  • Ascent Colorado – Ascent Colorado is a community-based program that provides treatment and support to youth and young adults 15-29 years old who may be experiencing an early onset of psychosis.
  • The Colorado Office of Suicide Prevention, housed within CDPHE, leads the statewide Gun Shop Project. In order to serve the entire state, the Office of Suicide Prevention funds ten grantees to visit gun shops, retailers, and firearm-related community events to share suicide prevention resources and information about temporary safe storage options in the case that someone is experiencing suicidal despair. In addition to the ten community grantees leading this work locally, OSP staff and the West Slope VA outreach additional counties, so that the project reaches statewide. Since 2020, the Gun Shop Project has conducted more than 2,500 visits to gun shops and community events statewide.
