Community Spotlight West Mountain

The Garfield County Public Health Department in Colorado is dedicated to promoting health and preventing disease within the community. They offer a wide range of services, including immunizations for children and adults, flu vaccinations, communicable disease surveillance, emergency planning, and food safety and licensing. They also provide resources for managing chronic diseases, mental health support, and substance use disorder treatment. Additionally, the department is involved in environmental health initiatives such as air quality monitoring and radon testing.

One of their key programs is the Women, Infants, and Children (WIC) program, which provides nutritional support to pregnant women, new mothers, and young children. WIC offers free healthy foods, breastfeeding support, nutrition education, and referrals to other health and social services. This program aims to ensure that families have access to the resources they need for healthy growth and development.

SimpliFed is a Maternal Care at Home model, centered on exceptional patient experience while driving strong clinical outcomes and reducing cost (at least $1-6K per patient based on actuarial claims analysis). We specialize in lactation and baby feeding support as well as high-risk condition monitoring for maternal mental health, social drivers, and more. Our providers engage with patients early and our care navigators guide patients through the entire maternal episode of care, offering 24×7 support via our secure virtual platform.

By engaging early and supporting often, we are moving the needle on key metrics, such as the following outlined below.

2% No show rate (national average 18.8%)
87% of our patients feed with breast milk for at least 3 months (national average is 69%)
2.6% ER postpartum utilization (national average 7.6%)

Delta County Public Health’s Clinical Services program plays a vital role in safeguarding the health and well-being of the community. Focused on providing accessible, affordable, and high-quality care, this program offers a variety of essential health services aimed at prevention, education, and early detection.

Key Services Offered:

  • Immunizations: Protecting individuals and the community against vaccine-preventable diseases with childhood, adult, travel and immigration vaccines.
  • Reproductive Health: Comprehensive services, including family planning, birth control consultations, IUD placement, pregnancy testing, and counseling.
  • Sexually Transmitted Infection (STI) & HIV Testing and Treatment: Ensuring privacy and confidentiality while providing screenings, treatment, and education to reduce the spread of STIs.

Sharing Ministries

Sharing Ministries Food Bank has been serving the Western Slope since 1996. We provide supplemental food, personal hygiene products, and socks to those in need. We provide services to all people without regard to race, color, national origin, sexual orientation, gender expression, military status, religion, age, disability, political beliefs, marital or family status. We treat all people with the utmost dignity and respect.

Sharing Ministries has 3 food supplemental programs; Sharing Ministries Food Program, USDA Emergency Food Assistance Program, and the USDA Senior Supplemental Program. There are no qualifications for the first two programs and seniors over 60 are income-qualified through USDA. Clients can receive each program once per month. Sharing Ministries Food Bank provides staples, such as canned goods, pasta, rice, produce, dairy, meat, frozen items, etc.

Our hours for distribution are Monday through Friday, 9:30am-12:30pm. For more information contact the food bank at (970) 240-8385.

Funding Opportunities – 

  • Funding (due 10/1/2025) – The Colorado Department of Public Health and Environment (CDPHE) is pleased to announce the release of an Application for Reimbursement through the Prescription Accessibility Grant Program on December 11, 2024. The Prescription Accessibility Grant Program was created to award hardship grants to pharmacies that face financial barriers to complying with House Bill 24-1115. CDPHE will award approximately $65,000 to pharmacies, with a maximum award of $1,500 per individual grant recipient. Grant funds must be used exclusively to purchase equipment that creates accessible prescription labels. Eligibility for this funding is as follows:
    • Pharmacies registered and based in Colorado.
    • Independent pharmacies
      • An ‘independent pharmacy’ is defined as a prescription drug outlet privately owned by at least one licensed pharmacist with no ownership interest by or affiliation with a chain or publicly owned pharmacy.
    • Ineligible entities
    • Publicly traded pharmacies (Walgreens, CVS, etc) are not eligible for this funding opportunity.
    • Pharmacies that meet eligibility criteria and have purchased labeling equipment that complies with the new accessibility requirements are encouraged to apply for grant reimbursement. The application deadline is October 1, 2025. Review the application requirements and apply for grant reimbursement.
  • Funding  – Outdoor Recreation Legacy Partnership (ORLP) Program Funding –  The ORLP offers 50/50 matching grants to create new parks or revitalize existing parks in underserved urban neighborhoods. Grants can be used for land acquisition, new park development, or the substantial renovation of existing parks, to (re)connect people with the outdoors.   • Funding (applications accepted on an ongoing basis) – Hearst Foundations Health Grants. The Hearst Foundations provide grants for organizations working to address access to healthcare for high-need populations, healthcare professional shortages, evolving healthcare demands, and medical research.
  • Funding (applications accepted on an ongoing basis) – National Grassroots Organizing Program. Two-year unrestricted, general operating support grants of up to $30,000 per year, with an average grant size of $20,000 per year, to small, constituent-led grassroots organizations throughout the US and its territories.
  • Funding (applications accepted on an ongoing basis) – Economic Development Assistance. Funding to support the implementation of regional economic development, including public works projects and job creation projects. Past projects have included funding for water and wastewater projects in hospitals and healthcare facilities and road infrastructure projects that would support the expansion of healthcare facilities. Rural tribal and other underserved communities are of priority.
  • Funding (applications accepted on an ongoing basis) – Robert Wood Johnson Foundation. Grants for innovative projects that are working to build health equity, ensuring that everyone has access to the care they need and all families have the opportunity to make healthier choices.

  • Please join the Department of Family Medicine Community Engagement Program Monthly Meeting (DFM-CEP-MM) on Tuesday, February 18, 2025 from 12-1pm for an exciting group presentation from the Trailhead Institute, Regional Health Connectors (RHCs), and their partners. Come join Trailhead Institute to learn about the Regional Health Connectors (RHCs) and their work with community-clinical linkages, alongside the Colorado Public Health Works program, supporting community health worker apprenticeships across the state.   • Conference – Nourishing Our Communities: The Power of Indigenous Nutrition in Healthcare,” which is organized by Fort Lewis College Nutrition Department, Tumbleweed Nutrition, and SWCAHEC, remote and in person options available. Please see attached flyer!
  • Training – Special Olympics has created an online, go at your own pace, training for Community Health Workers to help educate CHWs on working with the IDD population. The training also has information on healthcare for the IDD community along with barriers that exist along with many statistics and information around health for individuals with IDD. Below is some information on accessing the training if this is something CDPHE would be interested in. For the Community Health Worker Training, this is how people can access it:
    • Visit ////elearn.specialolympics.org/health
    • At the bottom of the white box, click on “Register”
    • Fill out all fields, including email, name, preferred language, and time zone. Be sure to answer the question “Where are you located?” to select United States. Click “Next” in the bottom right.
    • Fill out all fields, including state, gender, area of focus, and major or program.
      • For Area of Focus, select Health Professions Education unless one of the other options applies to you.
      • For Major or Program, select your area of practice or study.
    • Once all fields have been filled out, click “Register” in the lower right corner.
    • After clicking register, there is a slight delay where an email is sent to confirm/verify the email address entered. This is usually a quick process, but the email may go to user’s spam folders, so if the email isn’t received in 1-2 hours, check the spam folder.
    • Once the account is all set up, either the “Community Health Worker Training” course will be an option listed in the recommended courses, or they will need to search for it. It looks like this:
  • Training – The Colorado Office of Suicide Prevention (OSP) is excited to announce we are now offering a virtual, two-day Brief Cognitive Behavioral Therapy (BCBT) Workshop in collaboration with Anduril, LLC, exclusively for Colorado healthcare professionals! Two additional sections were added for 2025 due to the high demand and interest! The workshop will equip you with essential skills to effectively support individuals at risk of suicide. Learn how to establish a collaborative relationship, assess risk, and implement proven interventions for crisis management and emotion regulation.  The format of the training will be two 8-hour live online sessions. Attendance is mandatory for both days.  Training Dates: OSP has 4 opportunities to participate, applicants will be notified if they are accepted at least 3 weeks prior to training. The available dates in 2025 are as follows:
    • March 17 and 18
    • April 10 and 11
    • April 28 and 29
    • Time: 8:00 AM – 4:00 PM MST (both days)
    • Certification: Attendance on both consecutive days is mandatory for certification. Failure to not actively participate during both days will result in no certificate.
