CRN Trailblazers: Nadia Vargas
Community Resource Coordinator at Mountain Family Health Centers
Nadia is the Community Resource Coordinator at Mountain Family Health Centers. She joined MFHC 7 years ago as a Medical Assistant but transferred to Complex Care where she began helping patients with Social Determinates of Health Needs. As Coordinator, she manages the incoming referrals from community partners who need to set up their patients with a Primary Care Provider, Counseling, or Dental needs. Her team also works with patients who have identified needing assistance. They help search for resources that can alleviate their needs, and most of the time, those resources are found in CRN. Nadia also works closely with other community agencies to learn more about their programs and build a close relationship with them to better support her patients.
When she’s not at work she enjoys spending time outdoors with her family and visiting new places.
