2-1-1 powers CRN’s Resource Directory.
Not sure where to send your patient/client for the resources they need?
Western Colorado 211 is the perfect resource to refer clients when you are unsure where to send them for the services they need. You can send an electronic referral through CRN directly to them. You’ll notice they always show up at the bottom of your search results in the Resource Directory. PRO TIP: Mark them as a favorite for even easier workflows! ->
Their Information and Referral Specialists can point your client in the right direction for the services they need. Western Colorado 211 is able to provide accurate and comprehensive information and referral services to individuals and families across all different backgrounds and financial situations.
Every hour of every day, hundreds of people need essential human services. They are looking for training, employment, food pantries, help for an aging parent, addiction prevention programs for their teenage children, affordable housing options, support groups, and ways of becoming part of their community. 211 allows people to give help and to get help.
2-1-1 is currently partnering with Hilltop’s Health Access program.
With the public health emergency ending, Health Access is helping people find out if they are at risk of losing their Medicaid benefits, and if they are, what they can do to prepare and possibly find alternative affordable health insurance coverage.
Anyone can call 211 to be directly connected to a Health Access worker who can help them.

As a nationally-recognized trailblazer in the SDoH information exchange space, our CRN platform was called-out as a crucial resource for other health organizations interested in developing similar initiatives in their communities.
The Office of the National Coordinator for Health Information Technology (ONC) is the federal government’s lead agency for healthcare technology. ONC’s SDoH Information Exchange Toolkit also highlighted QHN for our expertise in the technical infrastructure and data standards that makes our whole health information exchange so robust.
QHN is proud to partner with our over 500 health, social, and behavioral partners to realize the very best in information exchange standards, and we are excited to continue to find new ways to save our participants time, money, and even lives in our western Colorado community.

SimpliFed
We provide virtual, judgment-free breastfeeding/lactation and baby feeding support, centered on parents’ baby feeding goals and can help with breastfeeding (latch assessment, supply concerns, nipple/breast pain, infant oral assessments, etc), pumping (sizing, use, setting up a pumping schedule to maintain supply, etc.), formula prep and use, combination feeding, and more.
Our certified lactation providers deliver virtual, judgment-free breastfeeding/lactation and baby feeding support, centered on parents’ baby feeding goals and can help with breastfeeding (latch assessment, supply concerns, nipple/breast pain, infant oral assessments, etc), pumping (sizing, use, setting up a pumping schedule to maintain supply, etc.), formula prep and use, combination feeding, and more.
Health First Colorado, Rocky Mountain Health Plans and Colorado Access members have unlimited access to comprehensive breastfeeding and baby feeding support with SimpliFed throughout the baby feeding journey, starting in pregnancy. – These benefits are covered under the mother/ birthing parent or infant.
SimpliFed is an in-network provider with Aetna & TRICARE West; patients have access to 6 covered lactation appointments, starting in pregnancy.
No prior authorization or copays are required. We continue to expand our relationships with other health plans.

Charlie Health’s accessible, completely virtual Intensive Outpatient Program (IOP) is designed for those 11-33 who need more than one-time-per-week therapy. Our IOP consists of curated and customized groups, individual therapy, and family therapy. With our virtual and accessible platform, healing is possible from home. Our trauma-informed, masters-level clinicians use evidence-based therapy including DBT skills, CBT, ABFT, EMDR, and others. Our expert, creative arts therapists guide our patients through music, art, dance, movement, and meditation therapies. We offer specialized tracks based on the needs of our patients, including BIPOC, LGBTQIA+, trauma, substance use, and others. We work with all major health plans, including
Medicaid, to ensure that treatment is accessible and affordable. We take ALL Commercial and Medicaid plans in CO including Tricare and Kaiser. We can work with individuals and families with depression, trauma, mood disorder, anxiety disorders, self-harm, substance use disorders, gender dysphoria, and suicidal ideation.
