The LOR Foundation’s 2025 Field Work initiative funds innovative ideas that improve rural mental health.
Field Work is the LOR Foundation’s search for innovative answers to persistent problems that plague rural communities. Our goal with each Field Work initiative is to get money quickly into the hands of the people on the ground—in communities where LOR works or in other rural parts of the region—who have innovative ideas. This year, LOR is focusing on finding innovative solutions that improve mental health or social well-being in rural communities. Funding proposals can range from $1,000 up to $25,000 for a project. LOR will begin accepting project proposals May 12 and will close applications on June 6. Applicants will be notified by late June.
- Who Can Apply: LOR’s 2025 Field Work initiative is open to individuals, local governments, hospitals, schools, nonprofits, for-profit organizations, chambers of commerce, libraries, and other organizations in Colorado, Idaho, Montana, New Mexico, or Wyoming.
- What Can Be Funded: Projects that improve outcomes in the following areas are eligible:
- Preventing and responding to mental health concerns
- Increasing access to and/or awareness of mental health and social well-being services
- Deepening meaningful social connections for the specific purpose of improving mental health outcomes in a community

Soulful Recovery is the premier virtual addiction treatment provider in Colorado. We serve ages 13-64 and specialize in intensive outpatient programming as well as outpatient programming (see below for further details). Dino Peds
At Grand Junction Gastroenterology, we’re a group of physicians specializing in the care of digestive and liver related conditions. We are a referral-based specialty clinic.
Our physicians are board-certified Gastroenterologists. We do our best to provide advice, testing, and management for outpatient and inpatient adults with symptoms related to the esophagus, stomach, liver, gallbladder, pancreas, small bowel, and colon. We also provide GI related screening and preventative care for adults without symptoms.

Project 1.27 acts as a bridge between the church community and kids in foster care who need families. Through the training and support of foster and adoptive parents, families are better prepared and encouraged throughout their journey. Project 1.27 also partners with local churches and agencies in order to recruit more families and smooth the way for parent certification.

Funding Opportunities
- Funding (due 9/30/2025) – Grants to support health programs, average grant size is $10,000 – $20,000. Prioritizes communities in southeast and southwest Colorado.
- Funding (due 10/1/2025) – The Colorado Department of Public Health and Environment (CDPHE) is pleased to announce the release of an Application for Reimbursement through the Prescription Accessibility Grant Program on December 11, 2024. The Prescription Accessibility Grant Program was created to award hardship grants to pharmacies that face financial barriers to complying with House Bill 24-1115. CDPHE will award approximately $65,000 to pharmacies, with a maximum award of $1,500 per individual grant recipient. Grant funds must be used exclusively to purchase equipment that creates accessible prescription labels. Eligibility for this funding is as follows:
- Pharmacies registered and based in Colorado.
- Independent pharmacies
- An ‘independent pharmacy’ is defined as a prescription drug outlet privately owned by at least one licensed pharmacist with no ownership interest by or affiliation with a chain or publicly owned pharmacy.
- Ineligible entities
- Publicly traded pharmacies (Walgreens, CVS, etc) are not eligible for this funding opportunity.
- Pharmacies that meet eligibility criteria and have purchased labeling equipment that complies with the new accessibility requirements are encouraged to apply for grant reimbursement. The application deadline is October 1, 2025. Review the application requirements and apply for grant reimbursement.
- Funding (11/1/2029) – Outdoor Recreation Legacy Partnership (ORLP) Program Funding – The ORLP offers 50/50 matching grants to create new parks or revitalize existing parks in underserved urban neighborhoods. Grants can be used for land acquisition, new park development, or the substantial renovation of existing parks, to (re)connect people with the outdoors. - Funding (applications accepted on an ongoing basis) – Hearst Foundations Health Grants. The Hearst Foundations provide grants for organizations working to address access to healthcare for high-need populations, healthcare professional shortages, evolving healthcare demands, and medical research.
- Funding (applications accepted on an ongoing basis) – National Grassroots Organizing Program. Two-year unrestricted, general operating support grants of up to $30,000 per year, with an average grant size of $20,000 per year, to small, constituent-led grassroots organizations throughout the US and its territories.
- Funding (applications accepted on an ongoing basis) – Economic Development Assistance. Funding to support the implementation of regional economic development, including public works projects and job creation projects. Past projects have included funding for water and wastewater projects in hospitals and healthcare facilities and road infrastructure projects that would support the expansion of healthcare facilities. Rural tribal and other underserved communities are of priority.
- Funding (applications accepted on an ongoing basis) – Robert Wood Johnson Foundation. Grants for innovative projects that are working to build health equity, ensuring that everyone has access to the care they need and all families have the opportunity to make healthier choices.
Webinars, Trainings & Conferences

- Training – May 14th 10am-1pm MST – Join our upcoming workshop to explore the Learning Agenda for Systems Change, a tool that helps health departments connect community health data to workforce development planning in a way that fosters system change. Learn how to use transformative learning to drive systems change and better integrate assessments like CHAs, CHIPs, and Strategic Plans.
- Training – May 20 and June 12th 9-12pm – Join us for a 3-hour workshop to explore honoring culture through cultural competency, sensitivity, humility, and safety. Engage in self-reflection, group discussions on health equity, unconscious bias, and privilege, and compare Western and Indigenous values. Learn strategies for addressing conflict through restorative conversations and reflect on the medicine wheel’s quadrants to enhance well-being. The workshop will be offered on two dates; please select the one that works best for you.
- Training – May 28th 11am – AI for Small Business ( English)
- Training (free!) – June 17th and 18th – Rocky Mountain HIDTA and ADAPT are co-hosting a 2-day Youth Prevention Institute at Pikes Peak State College. The purpose of the event is to equip professionals with knowledge and skills to cultivate connection and health-promoting relationships with youth as protective factors across systems of influence. The intended audience is prevention professionals, coalitions, law enforcement, educators, caregivers, and other trusted adults in the community committed to youth wellbeing. It is free to attend and CE’s are offered. See attached flyer.
- Training Opportunity for Young Adults: Build a Career in Population Health – HealthTeamWorks Population Health Career Lattice (PHCL) program offers clear, structured pathways for individuals to advance from entry-level to experienced roles in Community Health Work, Care Management, and Clinical Health Coaching. The program is designed to equip participants with essential skills to support their communities while building meaningful, sustainable careers in healthcare. Now available in Colorado, Iowa, and Missouri, the PHCL program is designed to support individuals who are passionate about making a difference in underserved communities. Eligible participants include:
- Youth aging out of foster care
- Graduates of alternative schools entering the workforce
- Young adults (ages 17–24) living at or below the poverty level
- Individuals committed to serving underserved populations in Iowa, Colorado, and Missouri
- Participants receive professional training, tuition scholarships, course completion stipends, and may be eligible for additional social support based on assessed need.

- Webinar Series – Rocky Mountain Health Plans invites you to join us for our Behavioral Health Skills Webinar series. These virtual events are free of cost to attendees and features Colorado-based and national experts. These events are designed for primary and specialty care teams, community mental health and independent behavioral health providers, and community organizations who assist with access to behavioral healthcare. Held on the third Tuesday of each month from 12:00-1:00 PM MST starting in March, these webinars will explore timely and impactful topics in behavioral health.

- ACC Phase III Attribution fact sheet: HCPF has made several updates to the attribution methodology for Phase III after receiving significant feedback about the current methodology in ACC Phase II. Learn more about those changes in this fact sheet.
- ACC Phase III PCMP Payment fact sheet: HCPF is introducing an updated primary care alternative payment model structure for all PCMPs that integrates programs previously operated separately by HCPF and the RAEs. The result is a single, comprehensive primary care payment structure that aims to elevate the quality of primary care services provided to Health First Colorado members.
- Medicaid Tracking – The House passed a budget resolution that paves the way for Republicans to begin negotiating how to pay for tax cuts by possibly reducing Medicaid spending by up to $880 billion or more over 10 years, along with other spending cuts. With 83 million people covered by Medicaid across the United States, including nearly four in 10 children, and six in 10 nursing home residents, Kaiser Family Foundation is tracking the Medicaid financing debate closely, and you can find all their resources, including their policy analysis, latest polling, and news stories on the website here.
- Cannabis Prevention Education Online Training Suite – The Cannabis Prevention Education Online Training suite is also part of the Cannabis Prevention Education project and is a great resource if you haven’t check it out yet or need a refresher. There are several modules to support the work you are doing in your community spaces surrounding prevention and working with young people. The paraphernalia module has also undergone a refresh and has some new content. Session material from session #1 and session #2 of the 2025 CoP as well as the CO Community Champion 2025 report is also available in both English and Spanish under the ‘Resources’ Learning Tile.
- Community Coaching w/ Metanoia – You can also find information about free community coaching that is also offered by Metanoia through this project in the Cannabis Prevention Education Online Training Suite. Don’t forget to check out the ‘Resources’ Learning Tile to see the CO Community Champion 2025 Report. If you and/or your community is curious in learning more about community coaching strongly encourage you reach out! Metanoia will also be offering a coaching demonstration workshop at the upcoming 2025 Shared Risk and Protective Factors Summit taking place in both Grand Junction (March 21st-22nd) and Pueblo (May 21-22nd).
