Beneficent connects seniors and disabled individuals to funds that pay for the high-quality care our industry partners provide. They have a 100% application success rate for the programs covering the costs of assisted living, adult day care, non-medical home care, assisted living, and skilled nursing facilities. They serve the whole state of Colorado.

Beneficent has been privileged to successfully assist thousands of families and individuals through asset preservation while coping with long-term care costs. They consider the trust extended to them over the last 20-plus years with their community partners and clients to be among their greatest honors.

Healthier Together Summit

Join us for a day of fantastic speakers and connecting for better health.

In 2024, QHN will celebrate 20 years of community collaboration by connecting and innovating healthcare, behavioral, and social organizations to provide crucial information that ensures people get the care and services they need quickly and efficiently.

We are looking forward to reuniting to ignite creativity and exploration, foster lively discussions that advance healthcare and address the social determinants of health, and share some laughter and successes.

We will hear from industry leaders like Kim Bimestefer (HCPF) and Lisa Bari (Civitas), as well as Craig Zablocki, who will guide us through what it means to transform through vulnerability and authenticity. There will also be a robust resource fair – an opportunity for you to highlight your good work and see the incredible resources that are offered in western Colorado – along with several thought-provoking Hot Topics sessions.

SAVE THE DATE! Thursday, April 25th, 2024. Registration opens on January 25th.

There will be no CRN April Collaborative. Instead, please join us for the Healthier Together Summit. Scholarships and complimentary tickets are available for CRN Network Partners and other non-profit organizations. You will also have the opportunity to bring a booth to the Summit’s resource fair.

Parent and Family Wellness Center

The Parent and Family Wellness Center provides psychotherapy and medication evaluation and management. While we do have an office in Boulder, our practice serves individuals, couples, and families throughout Colorado through telehealth. Our specialty is perinatal mental wellness, but we work with a wide array of clients who fall well outside that category (including men). We offer couples and individual counseling, parent-child dyadic therapy (in-person only), and we work with teens.

To the best of our abilities, we are committed to making high-quality mental health care accessible to all. We have a Spanish-speaking bi-cultural clinician on staff. In some counties, we partner with grant agencies to provide vouchers for free care to those who are uninsured or underinsured.

We offer free perinatal wellness check-ins, which can be helpful for those who are hesitant about receiving mental health support. We also offer monthly free workshops on PMAD prevention.

We provide gender-affirming letters of support for hormone-affirming therapy (HAT), gender-affirming surgeries (GAS), and other mental health support. Our providers specialize in trauma and EMDR, diversity issues, somatic work, and allyship. Our team also incorporates a variety of therapeutic modalities including CBT, (cognitive behavioral therapy), Motivational interviewing, EFT (emotional freedom techniques), EMDR (eye movement desensitization and reprocessing), IFS (internal family systems), SFT (solution-focused therapy), DBT (dialectical behavioral therapy), and parts work. We provide in-office and telehealth services for client convenience.

Access to Nutritious Food

Community Food Bank is an independent nonprofit serving Mesa County for 45 years. Our mission is to provide a short-term supply of nutritious food and access to available hunger relief resources to anyone in Mesa County experiencing need. While we are constantly changing and growing,

In-Person Shopping – The pantry is a self-choice pantry. Any community member can register with Community Food Bank and make an appointment to shop through the pantry up to 1x/week, 24 possible visits per year. No prerequisites. No ID required. Our pantry features fresh fruits & veggies, milk, eggs, meat, and bread in addition to shelf-stable foods. Clients go through the warehouse with a shopping cart and choose the items that work best for them. This model emphasizes client dignity while reducing food waste.

Home Delivery – 1x/monthly food box delivery for qualifying community members without a proxy to pick up food, no transportation options, and physical disabilities or medical conditions that inhibit their ability to access the pantry. Ongoing monthly deliveries require a referral from a medical provider or case manager. Both fresh and nonperishable food items are provided in deliveries. While clients can indicate food allergies and/or cooking limitations, exact foods included are determined by our current inventory. We encourage everyone to access our in-person pantry if able to ensure they can select the foods they prefer and so we can reserve the delivery program for our most vulnerable community members.

Resource Navigation – Our team can assist community members to submit a SNAP application, update existing SNAP applications, or simply provide more information and guidance concerning the program. We can also help families apply for the WIC program. Our navigators are dedicated to helping clients find additional local resources to better improve their lives.

Connecting Your Community

All Points Transit is a 501c3 non-profit organization providing public transit services for seniors, people with disabilities and the general public in communities throughout Montrose, Delta, San Miguel and Ouray counties.

The mission of All Points Transit is to provide safe, reliable and accessible transportation for all, supporting continued independence and enhanced quality of life. The vision is to deliver and operate a transportation system that emphasizes safety, environmental sensitivity, quality, and accessibility through the efficient movement of people, enhancing the economic vitality of the region. All Points Transit also provides public bus routes in Montrose with our Blue and Orange routes, we also connect to Delta with our MOD route, which stops at the Montrose Airport every two hours.

Over 30 years ago,Montrose County residents began providing transportation for their elderly and disabled friends and neighbors to doctors’ appointments, senior meals, and other needed services.  In 1989, the Montrose County Senior Citizens Transportation officially incorporated and was approved for a 501c3 status in April of 1990.  Since then, seniors and people with disabilities have been receiving transportation services within Montrose County, including the communities of Montrose, Olathe, Nucla, Naturita, Norwood, Paradox, Bedrock and Redvale.

In 2008, All Points Transit expanded to provide service in Delta County and now also serves the communities of Delta, Cedaredge, Hotchkiss, Paonia, Orchard City and Crawford.

Because of the overwhelming demand for transportation in the City of Montrose, All Points Transit launched the Montrose City Bus in August of 2010.

Webinars, Trainings & Conferences

  • Training – Age-Friendly Public Health Systems (AFPHS) Initiative offers an “AFPHS Champion” designation to empower and encourage more leaders to champion healthy aging. This recognition honors public health professionals who are committed to expanding their knowledge about healthy aging and want to lead their departments and organizations in becoming age-friendly. Anyone can become a Champion! Reaching this status is simple: individuals who attend or view at least six AFPHS monthly trainings will be designated as AFPHS Champions.   • New Screening, Brief Intervention, and Referral to Treatment (SBIRT) Trainings!  November and December training dates are posted! This includes trainings for SBIRT skills, motivational interviewing, and the new SBIRT implementation 101 series! Screening assists providers in identifying and intervening with patients at risk for health problems due to alcohol and other drug use. Contact SBIRTinfo@peerassistanceservices.org to schedule a training for your organization.
  • The Naloxone Project (TNP) Has Expanded into Health Clinics! In an effort to get naloxone access to every community in Colorado, The Naloxone Project (TNP) has expanded into health clinics. After working with 108 Emergency Departments and 50 Labor & Delivery rooms across the state, TNP received requests from clinics to receive naloxone for patients who are uninsured, underinsured, have poor access to health care, and patients at risk of opioid overdose. In March 2023, TNP received 6,000 doses of Narcan through the Colorado Department of Public Health and Environment’s (CDPHE) Naloxone Bulk Fund. Since March, 5,442 of the 6,000 doses have been distributed to 53 clinics across Colorado. A diverse group of clinics have enrolled; primary care, OB/GYN, behavioral health, dental, and orthopedics. TNP is honored to support these clinics in distributing this life-saving drug to their communities. The goal is to enroll 45-50 more clinics in the program with this second donation of naloxone.     • Additionally, TNP is hosting a Harm Reduction for the Holidays Event! Join them on Dec. 14, 2023, from 6:30 to 9:30 p.m. This family-friendly holiday event aims to get harm-reduction tools into the hands of everyone. With the generosity of various organizations, we will be stuffing stockings with a variety of harm reduction and prevention tools, including our fabulous TNP-branded naloxone keychains. This event will also host vendor tables selling small goods to round out those stockings, small bites, live music, and seasonal drinks for all ages. Get your tickets here!
