Healthier Together Summit

Join us for a day of fantastic speakers and connecting for better health.

In 2024, QHN will celebrate 20 years of community collaboration by connecting and innovating healthcare, behavioral, and social organizations to provide crucial information that ensures people get the care and services they need quickly and efficiently.

We are looking forward to reuniting to ignite creativity and exploration, foster lively discussions that advance healthcare and address the social determinants of health, and share some laughter and successes.

We will hear from industry leaders like Kim Bimestefer (HCPF) and Lisa Bari (Civitas), as well as Craig Zablocki, who will guide us through what it means to transform through vulnerability and authenticity. There will also be a robust resource fair – an opportunity for you to highlight your good work and see the incredible resources that are offered in western Colorado – along with several thought-provoking Hot Topics sessions.

Reclaim your balance

We are a local 501c3 non-profit that helps and supports the unhoused by providing tents, sleeping bags, winter clothing, hot meals and backpacks with hygiene supplies. We have a recovery program that facilitates recovery groups and face-to-face peer support inside Garfield and Eagle County jails. When inmates are being released, we meet with them and help connect them to other community partners and help them rebuild a healthy community around them. Rise Up Recovery also will be starting on February 17th a spanish speaking woman’s only recovery group and a woman’s only English speaking recovery group. Saturdays from 11:00am to 1pm both at the Glenwood Library. We are Colorado Peer and Family Specialist that have a heart for people that are struggling with mental health and substance use. Mountain Safe Response

Mountain Safe Response is a safe and reliable entry point for individuals to be directed towards available resources, improving outcomes for crime and trauma victims, deterring offenders from future crime, rehabilitating offenders, and reducing violent crime.

Mountain Safe Response serves as a basecamp where individuals and organizations can access a wide range of tools and support. We are a central connection point, ensuring that everyone has the necessary resources to overcome their obstacles as we work through crises together.

You Matter, We Care

SummitWest Care is a not-for-profit home care agency with over 20 years of experience in helping people of all ages remain in their homes as they age and heal. Our medical and non-medical services provide holistic and compassionate care to those older people, people with disabilities and chronic illnesses, and to infants who just went home from the Neonatal Intensive Care Unit. Our programs include both medical and non-medical services for people of all ages.

Medical services include Skilled Nursing, Physical/Occupational/Speech Therapy, CNA’s, and Medical Social Worker. We specialize in wound care, post-operational care, diabetes management, IV therapy, medication management, chronic disease management, tube feedings and much more. To further assist older adults and promote independence, we have implemented Telehealth and Remote Patient Monitoring and Lymphedema treatments.

The non-medical services, also known as In-Home Support Services, provide care, training, and resources to assist individuals with their daily living needs. People in this program are elderly, disabled, and/or children with disabilities. Homemaking and personal care services are based on the client’s individual needs and include duties such as light housekeeping, grocery shopping, meal preparation, stand-by-assists, medication reminders and more. Additionally, SWC offers personal safety devices with GPS and emergency call button capabilities. Through this program, CNA parents can provide care for their disabled children and get paid for it. Clients who have family or friend caregivers can have them onboarded by SWC to get paid for caregiving. We provide training, resources, and support for all caregivers.

The Parent and Family Wellness Center provides psychotherapy and medication evaluation and management. While we do have an office in Boulder, our practice serves individuals, couples, and families throughout Colorado through telehealth. Our specialty is perinatal mental wellness, but we work with a wide array of clients who fall well outside that category (including men). We offer couples and individual counseling, parent-child dyadic therapy (in-person only), and we work with teens.

To the best of our abilities, we are committed to making high-quality mental health care accessible to all. We have a Spanish-speaking bi-cultural clinician on staff. In some counties, we partner with grant agencies to provide vouchers for free care to those who are uninsured or underinsured.

We offer free perinatal wellness check-ins, which can be helpful for those who are hesitant about receiving mental health support. We also offer monthly free workshops on PMAD prevention.

Resources, to the power of 10 

Community Living Services at Region 10 is the Area Agency on Aging serving adults 60 and above. We are funded through the Older Americans and Older Coloradans Act to provide services and support to older adults so they can remain in their home. Services include information, assistance and referrals, transportation, nutrition programs, caregiver support, legal assistance and in-home services such as caregiver respite, homemaker and personal care. We also assistance with public benefits applications. The ADRC (Aging and Disability Resources for Colo) serves disabled adults aged 18-60, we assist in finding resources and applications and currently do not have funding for direct services under this program. The long-term care ombudsman program advocates for the residents in skilled nursing and assisted living communities. We are also home to the SHIP (state health insurance assistance program) and can assist with questions or enrollment into Medicare plans. RSVP (Retired Senior Volunteer program) recruits volunteers to assist with home delivered meals, transportation to appointments, food pantry support and much more. We have over 300 registered volunteers working in over 30 stations. Emphasis on programs that serve older adults is the priority for this program.

Star Light Home Health is a Medicare Certified Agency offering in-home skilled services following physician’s orders. We offer Skilled Nursing, Physical Therapy, Occupational Therapy, and Bath Aids for our homebound patients. (Speech Therapy and Social Worker coming soon). We are contracted with Medicare, Medicaid, and the Veteran’s Administration.

We are accepting referrals for patients that have Medicaid, Medicare, and VA.

Funding Opportunities – 

  • Funding (no deadline) – Building and Bridging Connections Funding
  • Loans (no deadline noted) – Rural Community Facility Loans
  • Funding (open for non-binding letters of intent) – Making Care Primary Model
  • Funding (no deadline noted) – Colorado County Jail Recognition Awards. Nominations are now open for a one-time $50,000 grant award to support jails to continue work in medications for opioid use disorder (MOUD) and connections to the community upon release. These awards are intended to recognize county jails within Colorado that have shown excellence or demonstrated progress in this work. Eligible nominees are exclusively limited to Colorado county jails. Nominators can include anyone from the public. The DOL encourages nominations from peer jail staff, sheriffs, individuals in the community, and individuals who have been incarcerated.
  • Funding (due 2/20) – Community Connect Broadband Grant Program. Supports broadband deployment into rural communities where it is not yet economically viable for private sector providers to deliver service. Broadband service should benefit residential and business customers in a service area, as well as critical community facilities, such as healthcare, public schools, fire stations, and libraries.
  • Funding (due 2/26) – Geriatric Workforce Enhancement Program (GWEP). The purpose of the GWEP is to educate and train the health care and supportive care workforces to care for older adults by collaborating with community partners. Applicants must maximize patient and family engagement to address care gaps and improve health outcomes for older adults by integrating geriatrics with primary care and other appropriate specialties using the Age-Friendly Health Systems Framework.
  • Funding (due 3/15, with registration on portal by 3/1) – Community Resilience Initiatives. These funding opportunities address The Colorado Trust’s strategic plan focus areas of food, housing, and mental and behavioral health. Register now for one of three pre-application webinars taking place February 9. For each initiative’s webinar, go to the website and click on the initiative of interest to register.
  • Funding (due 3/6)- 2024 AARP Community Challenge. These grants are meant to provide support to fund quick-action projects that can help communities become more livable for people of all ages. The AARP Community Challenge will be accepting applications for flagship grants, capacity-building microgrants, and demonstration grants.
  • Funding (extended through May 31, 2024) – COVID-19 Health Equity and Immunization Funding Opportunity.

  • Webinar: Navigating Rural Realities: Addressing Substance Use and Aging in Agricultural Communities – Thursday, February 22, 11-12pm MT. Join us for an insightful presentation that delves into the unique challenges faced by aging individuals in rural agricultural communities, where the intersection of substance use and the aging process pose complex problems. We will examine the social, economic, and healthcare factors that contribute to substance use among older adults in the agricultural sector.
  •  Webinar: Implementation Facilitation to Enhance Implementation of Evidence-based Suicide Prevention StrategiesWednesday, March 6th, 11-12pm MT. Presenter: Dr. Sara Landes. This presentation will focus on how implementation facilitation strategies have been applied to enhance the implementation of suicide prevention interventions within the Department of Veterans Affairs. The presentation will include an overview of the research design, the facilitation strategy, the facilitation process, and outcomes.
  • Webinar: Introduction to Capital Resources for Financing Rural Health Infrastructure and Community Development Projects – Wednesday, March 6, 12:30-2:00 pm MT. In this session participants will unwrap the basics of federal, state, and private sources of capital for rural health and prosperity and learn how capital resources are used in rural settings to achieve community health improvement goals.  Participants will learn about how projects are developed, and the role of the U.S. Department of Agriculture and U.S. Small Business Administration and other sources of financing.
  •  Envision: You recently announced a new training grant program geared toward behavioral health providers working with LGBTQ+ individuals. This opportunity is made possible through a state grant opportunity and aims to increase providers’ understanding of LGBTQ+ history, expand knowledge of inclusive language, and explore best practices for providing affirming care to LGBTQ+ individuals. Individuals interested in the training program can complete this interest form, learn more about the training, and send questions to training@envision-you.org.
  •  Training: Age-Friendly Public Health Systems (AFPHS) Initiative offers an “AFPHS Champion” designation to empower and encourage more leaders to champion healthy aging. This recognition honors public health professionals who are committed to expanding their knowledge about healthy aging and want to lead their departments and organizations in becoming age-friendly. Anyone can become a Champion! Reaching this status is simple: individuals who attend or view at least six AFPHS monthly trainings will be designated as AFPHS Champions.
