There are some important icons that pertain to organizations in the CRN Resource Directory. These icons can help you quickly identify important information about the organization and its ability receive referrals from the system:

A handshake with double arrows below indicates that the organization is a Network Partner and receives electronic referrals in CRN. Unless it is stated (Limited Referrals Accepted) this organization has users in the system, and you can send an electronic referral to them in CRN.

A handshake with no arrows indicates that the organization is a Network Partner but does not receive electronic referrals in CRN. There are users in the system, and while they may make outgoing referrals, they do not receive them. We recommend that users still log/document a referral to this organization if done outside the system for informational or reporting purposes.

A broken link indicates that the organization is an Associate Partner and accepts email/fax referrals out of CRN. While a referral can be sent out via email or fax, the loop cannot be closed as there are no users in the system.

If there is no icon for the organization, this means that the organization is not participating in the network. Referrals can be made outside of CRN, and we recommend that users still log/document these referrals for informational or reporting purposes.

The Arc of the Central Mountains is committed to providing lifelong support for individuals with intellectual and developmental disabilities (IDD), ensuring that they and their families receive the guidance, education, and advocacy they need at every stage of life. Our services span across multiple settings, including schools, communities, and the criminal justice system, where we work collaboratively with educators, healthcare providers, community leaders, and professionals. By building strong partnerships, we strive to create inclusive environments where individuals with IDD have equitable access to services, education, and opportunities.

In schools, we help students with IDD navigate educational systems, advocate for necessary accommodations, and work they can access the general curriculum alongside their peers. In the community, we facilitate connections to essential resources, programs, and activities that promote engagement and personal growth. Within the criminal justice system, we advocate for fair treatment and work to protect the rights of individuals with IDD.

Through our collaborative approach, we aim to break down barriers, promote understanding, and ensure that individuals with IDD and their families are empowered to make informed decisions. Whether through advocacy in policy reform or personalized support at the individual level, The Arc of the Central Mountains is dedicated to creating pathways to a more inclusive and supportive society for all.

Local answers you can trust on Connect for Health Colorado Marketplace Insurance and more!

In 2013, Hilltop’s Health Access was established as a certified assistance site for Connect for Health Colorado to provide information and application assistance for all the health plan options available under the Affordable Care Act. Our trained staff provide unbiased information and assistance with the Marketplace and other health plan options. We provide information on all available plans, help you complete the application, determine if you qualify for tax credits, and can even help you with government programs like Medicaid and CHP+. Hilltop’s Health Access has an office in Grand Junction but can provide health coverage assistance for people in our neighboring western slope communities located in Mesa, Delta, Montrose, and Garfield counties. Assistance is provided in English and Spanish, there is no charge for our services, and there are health plan options offering financial assistance for those who qualify. Hilltops Health Access can also help with understanding how to use your health coverage, find doctors in your plans network, understand your explanation of benefits, even if your insurance plan is through your employer or not purchased through the marketplace. If you’ve got questions about health care coverage, we’ve got answers. Open Enrollment for health coverage for the year 2025 begins November 1, 2024, and ends January 15, 2025. There are no eligibility requirements, and through access to a sliding scale, we work with people who have no insurance or are underinsured. Services for people ages 3 and up.

Funding Opportunities

  • The Colorado Office of eHealth Innovation, in partnership with the Department of Health Care Policy and Financing and the Department of Public Health and Environment, is pleased to share that the application for the Regional Social Health Informational Exchange (SHIE) Hubs grants program is now open. The deadline to apply for this funding opportunity is November 1, 2024. All application materials and information can be found on OeHI’s website, here, and on the State’s procurement website, here. Please direct all questions to the OeHI team at gov_shie_rfa_oehi@state.co.us. Please note that all questions related to this funding opportunity must be received by October 16, 2024, at 11:59 pm, so responses can be shared with all potential applicants on the OeHI website.
  • Funding (due 10/15) – Colorado Health Foundation has two upcoming funding opportunities: Advocacy and Justice with Communities of Color and Building a Pipeline of Primary Care Providers to Serve Diverse Communities
  • Funding (Due 11/11 by 5pm) – Harm Reduction Grant Fund- RFA #29157030 | Department of Public Health & Environment (colorado.gov). Applicants are encouraged, but not required, to complete the intent to apply form. This form allows CDPHE program staff to estimate the number of applications and plan for an adequately staffed review committee. The form will accept entries until Monday, October 28th. Please email sam.bourdon@state.co.us with any questions. All questions will be answered publicly and updated regularly on the FAQ page linked in the RFA website.
  • Funding (due 11/13) – FY2024 Public Transportation on Indian Reservations (Tribal Transit) Program. The Federal Transit Administration has made available more than $9 million in competitive grants for federally recognized Indian Tribes and Alaska Native Villages, groups, or communities as identified by the U.S. Department of the Interior Bureau of Indian Affairs. Eligible activities include planning, capital, and operating assistance for tribal transit services in rural areas.
  • Funding (due 2/26/25) – Strategic prevention framework. The purpose of this program is to help reduce the onset and progression of substance misuse and its related problems by supporting the development and delivery of community based substance misuse and prevention and mental health promotion services.
  • Funding (applications accepted on an ongoing basis) – Hearst Foundations Health Grants. The Hearst Foundations provide grants for organizations working to address access to healthcare for high-need populations, healthcare professional shortages, evolving healthcare demands, and medical research.
  • Funding (applications accepted on an ongoing basis) – National Grassroots Organizing Program. Two-year unrestricted, general operating support grants of up to $30,000 per year, with an average grant size of $20,000 per year, to small, constituent-led grassroots organizations throughout the US and its territories.
  • Funding (applications accepted on an ongoing basis) – Economic Development Assistance. Funding to support the implementation of regional economic development, including public works projects and job creation projects. Past projects have included funding for water and wastewater projects in hospitals and healthcare facilities and road infrastructure projects that would support the expansion of healthcare facilities. Rural tribal and other underserved communities are of priority.
  • Funding (applications accepted on an ongoing basis) – Robert Wood Johnson Foundation. Grants for innovative projects that are working to build health equity, ensuring that everyone has access to the care they need and all families have the opportunity to make healthier choices.

In 2022, the Health Resources & Services Administration (HRSA) awarded Trailhead Institute, in partnership with Patient Navigation and Community Health Worker Training Program (PNCT) and The Alliance of Colorado Community Health Workers, Patient Navigators and Promotores de Salud (Alliance), nearly $3 million to implement a training and apprenticeship program to support Colorado’s community health worker workforce through 2025.   • Parent and Family Wellness Center Perinatal Wellness Check-In. The Parent and Family Wellness Center is offering free wellness check-ins to pregnant and postpartum folks throughout Colorado. These 20-30 minute video sessions with a specially-trained mental health clinician provide psychoeducation about PMADs (perinatal mood and anxiety and disorders) and help folks understand and mitigate their risk factors. During your free 1:1 video session, a clinician will help identify what stressors you may have, check-in on your relationships, and educate you about how pregnancy and postpartum can affect your mood.   • On Sept. 11, 2024, the Department of Health Care Policy and Financing (HCPF) issued the following intent to award for the four Regional Accountable Entity (RAE) contracts in Accountable Care Collaborative (ACC) Phase III:
    • Region 1 – Rocky Mountain Health Plans
    • Region 2 – Northeast Health Partners
    • Region 3 – Colorado Community Health Alliance
    • Region 4 – Colorado Access
    • Through Sept. 26, 2024, HCPF will be reviewing any protests submitted. Barring any sustained protests, we will begin working with each vendor to finalize and execute new contracts.
    • The ACC is the primary delivery system for Health First Colorado (Colorado’s Medicaid program). The ACC was created in 2011 to improve health care access and outcomes for Health First Colorado members. RAEs are responsible for promoting member health and well-being by administering the capitated behavioral health benefit, establishing and supporting networks of providers, and coordinating medical and community-based services for members in their region. Current contracts with the RAEs end on June 30, 2025. The next iteration of the ACC, referred to as Phase III, will begin on July 1, 2025.
    • ACC Phase III was designed through extensive stakeholder engagement. HCPF staff met with more than 5,700 participants, including members, providers, and representatives from community organizations, across 135 stakeholder meetings over the past 18 months to identify opportunities to improve the program. Phase III will build on the foundation set over the past 13 years to improve quality care, close health disparities, improve care access, improve the member and provider experience, and manage costs to protect member coverage, benefits and provider reimbursements.
    • Designing ACC Phase III has been a tremendous effort, and we are so grateful for the invaluable input from stakeholders that has helped us reach this point. ACC Phase III takes important steps to modernize our Medicaid delivery system, increase accountability and drive better outcomes for our members. However, awarding contracts is just one part of this process. Over the coming months, we will be working with internal and external partners to implement the policies and programs outlined in the RAE contracts. We will continue to post updates and other resources for members, providers, counties and other key partners on our website and through this newsletter.
  • CoPPCAP, the Colorado Pediatric Psychiatry Consultation and Access Program: Zero counties in Colorado have a sufficient number of child & adolescent psychiatrists for their populations. To address this severe workforce shortage, CoPPCAP was launched in September 2019.  CoPPCAP provides access to board certified child and adolescent psychiatrists for Colorado’s primary care clinicians for peer-to-peer consultations.  CoPPCAP aims to increase the ability and comfort of primary care clinicians to provide basic mental health assessment and treatment for their child and adolescent patients via consultation by licensed child and adolescent psychiatrists, education, and community resources.  All pediatric primary care and family medicine clinics qualify for services.  There is no cost to enroll in CoPPCAP and there are no insurance-based restrictions for consultation or requests for resources.  Please watch our brief informative video and visit our website.
  • The Colorado Sun featured an op-ed on the need for a unified public health response to firearm violence prevention in Colorado, featuring Jonathan McMillan from Trailhead.
  • PROSPER is your partner in perinatal mental health, this program aims to enhance healthcare professionals’ skill and capacity to conduct universal screening, assessment, and treatment within routine obstetric and primary care settings.  This is achieved primarily through real-time phone access to reproductive psychiatrists, and one-time face-to-face patient consultations for diagnostic clarity and to guide treatment in the obstetric or medical home.
